Chemicals of Interest Inventory

The Department of Homeland Security (DHS) requires the University to conduct a periodic screening threshold quantity survey to comply with Federal regulations. Please complete this form by entering the amount (in pounds) of each of the chemicals listed below which are present in your lab as of the current date (page can be printed out to assist in reviewing your inventory, but submit results using the online form). Note some fields require a response (denoted by an asterisk and in bold). For more information, a list of Frequently Asked Questions is available. Please submit the data to UB EHS no later than April 15, 2011. Specific questions can be directed to Tony Oswald, EHS Hazardous Materials Manager at 829-3301.
