Special Event Form

University Facilities charges for the delivery and pick up of the special event items listed below. Once the items are delivered, the department or organization are responsible for their safekeeping.  The cost for replacement of missing or damaged items will be billed to the department or organization.

**Please note:  University Facilities does NOT set up or breakdown tables and chairs.**

If space is reserved through Conferences and Special Events, please call Liz Hladczuk at 645-2055.  If no reservation is placed with Conferences and Special Events, please fill out the form below.

Forms and Policies

Please send the RF & UBF Authorization form in addition to your special event request if you use RF or UBF accounts and funds.  State account holders need only fill out the special event request form below.
